Apply by Mail

Download, print, fill in, and send by mail a copy of the application form provided. Send to the Secretary of the O'Brien Foundation at the address below. You are responsible for having three letters completed by your referees and sent to the address indicated below. You are also responsible for ensuring receipt of a complete, official copy of all post-secondary transcripts.
